Most likely you have good downstream but bad upstream so the card can't stay paired. I had this happen plenty of times. Had to go through the guided setup until it repeated the problem. Replace the cable card as previously recommended, It'll help you out the most. May have 1 bad tuner, check all of them to make sure!

As soon as they pair it, the Tivo proceeds to run thru setup and load channel guide etc.. They say it all good and give it hald hour to finish loading everything. When it finally finishes I go to watch and nothing but V58 errors programming not authorized. Call Cox back and they say everything looks good, they will have to send Tech out next day.
