V58 errors everywhere. My least favorite thing in the world is to contact Comcast/Xfinity, but my hand was forced. Surprisingly, after about 25 minutes on the phone, they were able to get the CableCard to receive programming, all channels except the Premium channels, HBO, Showtime etc. Signal is bad or card is. Most likely you have good downstream but bad upstream so the card can't stay paired. I had this happen plenty of times. Had to go through the guided setup until it repeated the problem. Replace the cable card as previously recommended, It'll help you out the most. May have 1 bad tuner, check all of them to make sure!
